This works very well, and has all the formats unlike others.
The only thing I miss is an in-page button near the video that some other addons have. It would make usage more straight-forward and convenient.

I think this needs a whitelist too. For example on one website there's this really annoying page background ad link (which I constantly click accidentally), which since it's in js is there even if I hide the background in with the adblocker. But I need for the forum to work properly. (Where there aren't any background ads.) So I have to block/enable the site all the time.

This would be really could if the addon could browse useragentstring.com, to load UA strings from there. Because that site has up to date user agent strings.
Or an autoload a constantly updated xml list would be about as good too. But I don't know of any such lists...

It really needs an option to configure tool-tip delay.
Also the favicon could be showed based on domain name match and not by exact url match.

I was looking for a "Smart Middle Click" for firefox 4, which breaks it totally. Unfortunately this breaks it in the exact same way.
