When you reply to a mail sent to your personalized address (postfix/qmail/gmail extensions) such as john.doe+contactname@example.com, "From:" is replaced by your personalized address rather than your simple address.

This extension looks for email header "x-original-to", if not found then use address from "to" field if there is only one address.
