@ Thomas Bertels ,
Thanks for your answer. Australis has made the customise toolbars dialogue almost useless for me. A gazillion little pictures to guess about the meaning of. And just to make the game more challenging the pictures are all very, very pale grayish pastels on a light gray background. There is this really neat technology that has the potential to dramatically improve the usability of UIs. I hope the Mozilla folks will think about trying it although it may be kind of radical for them - it's called text. Not your doing of course. I searched and searched for your off button and I think I found it - an itty bitty x maybe? I dragged it toward the bar and it dissapeared about halfway there never to be seen again. But I installed Exit Button and that seems to serve the same purpose and makes a pretty good combo with this.

This review is for a previous version of the add-on (1.4.1-signed).  This user has a previous review of this add-on.

Thank you for your new review.
It seems there's something wrong with your Firefox, there should be text below each icon, either in the customize menu or in the new menu itself.
I suggest that you start Firefox in safe mode (by example by holding the shift key and then starting it - be sure to NOT click on Reset Firefox in the following dialog box).
Then if it temporarily fixes the problem, you can try and disable your add-ons one by one (and restarting Firefox each time) to find the culprit.
