Data is transmitted only to app.addy.io or to the self-hosted AnonAddy server you specify. If you are using app.addy.io, then its privacy policy (https://addy.io/privacy/) applies. If you are using an AnonAddy server you maintain, then your data is only going to your server. If you are using an AnonAddy server someone else maintains, you will need to check with them about their privacy policy.

No data is transmitted by this add-on to the author and maintainer of the add-on. The author/maintainer is unaffiliated with the creater of AnonAddy / maintainer of app.addy.io.
